Employment/Income
  • Employer addresses for the past two years.
  • Gross monthly income.
  • W-2's if available.
  • Year-to-date pay stubs.
  • Proof of any pensions, retirement, disability or Social Security.
  • Proof of any child support or alimony paid or received.
  • Proof of any income from rentals, investments, etc.
  • If self employed, two years 1040 tax returns.
  • If self employed, current year profit and loss statement.

Creditors

  • Each creditor's name, address and type of account.
  • Account numbers.
  • Monthly payments and approximate balances.
  • Amount of child care expenses.

Banking

  • Names and addresses of all bank or savings accounts.
  • Account numbers of all accounts.
  • Type of account and present balances.

Miscellaneous

  • List of assets in stocks, bonds, land.
  • Life insurance cash value (documented if used as a cash down payment.)
  • If you are selling a home, a copy of the sales contract.
  • Social Security number of all parties involved in the loan.
  • If you are a veteran, a Certificate of Eligibility and DD-214.
  • Cash or check to pay for the application fee.
